What the Job Involves
This role involves working within the Children with Disabilities Team to assess the needs of children with disabilities, develop and implement care plans, and provide essential support to families. You will collaborate with a multi-disciplinary team and external agencies to ensure the children receive the highest quality care and services tailored to their individual needs. The role requires regular visits, case management, and participation in meetings to monitor and review the progress of each child.
