You are an empathetic and resourceful Outreach Family Support Worker with a dedication to improving the lives of children and families. You are adept at building relationships, conducting assessments, and guiding families through complex processes to achieve positive change.

What the Job Involves

This role involves providing empowering interventions by coordinating support as the lead professional. You will conduct early help assessments, develop support plans, and chair team-around-the-family meetings to track progress and identify additional support needs. Direct work with children and young people will be vital to understanding and addressing their views and worries, ensuring interventions are effective in improving their long-term outcomes.
