What the Job Involves
This role involves working closely with hospital teams to facilitate discharge planning and coordinating referrals to boroughs of residence. You will liaise with hospitals such as Royal London, Newham, and Mile End to support patients requiring housing or social care upon discharge. A key focus will be reducing demand on hospitals and social care by providing effective housing solutions. You will also provide tailored housing advice and options, collaborate with multi-disciplinary teams, ensure compliance with statutory duties, maintain accurate records, and work proactively to prevent homelessness and secure sustainable housing solutions.
