You are an experienced Fostering Social Worker with a deep understanding of Staying Put arrangements. With your expertise, you excel in care planning and have a proven track record of supporting placement stability for young individuals post-18. You possess the confidence to challenge any drift and delay, ensuring the best outcomes for those in your care.
What the Job Involves
As a Staying Put Liaison Worker, you will support care planning for children and young people in various foster care settings. Engaging from when the young person turns 16, you will monitor the progress of care plans and facilitate information sharing to achieve Staying Put once they turn 18. Post-18, you will provide continuous support and guidance to former foster carers, visiting them every 12 weeks to improve the stability and security for the young person. This role aims to enhance quality and performance in the Fostering Service, ensuring a seamless transition to independence and sustained permanence with their foster families.
