What the Job Involves
You will work as part of an integrated team, conducting needs-led assessments, care, and support planning, and reviews. Your role includes providing guidance and information to adults and carers, preventing hospital admissions, and undertaking safeguarding enquiries. You will communicate complex information to stakeholders, participate in community initiatives, and make timely responses to allocated work, ensuring the highest standards of performance. Compliance with regulations and the promotion of self-determination and choice are key to your role.
Skills
- Social Work qualification, e.g., Degree in Social Work; Dip. SW or equivalent
- Registered with Social Work England
- Knowledge of relevant legislation: Care Act 2014, Mental Capacity Act 2005
- Strong communication and interpersonal skills
- IT proficiency, including Microsoft Office
- Organizational skills and ability to manage competing demands
- Experience with safeguarding enquiries and working with adults with complex needs
- Ability to prepare statements or reports for court proceedings (desirable)
- Best Interest Assessor training (desirable)
- Practice Educator award (desirable)
