What the Job Involves
The Outreach Family Support Worker will take whole case responsibility for providing high-quality support to children and families across various locations. You will act as a case holder for children with multiple support needs, completing assessments, coordinating plans, and ensuring a smooth transition between different levels of support. Key responsibilities include delivering targeted support, managing and prioritizing workloads, engaging with families to assess support needs, and utilizing de-escalation methods for challenging behavior. You will maintain thorough documentation and actively participate in supervision and developmental opportunities to enhance service quality.
