What the Job Involves
In this role, you will be responsible for providing high-quality outreach and home-based support to children and families using a whole-family approach. You will act as the case holder for children with varying levels of support needs and lead the coordination of support services to ensure positive outcomes. Your responsibilities include assessing family needs, developing tailored support plans, managing case files, providing de-escalation methods, and engaging with service users to foster effective partnerships. You will also liaise with other professionals and contribute to the ongoing development of services.
