What the Job Involves

As an Outreach Family Support Worker, you will provide high-quality, home-based Early Help support to children and families. Your role involves acting as a case holder for complex cases, undertaking early help assessments, and coordinating Team Around the Family meetings. You will deliver targeted support, manage smooth transitions between early help and social care, and utilize methods such as de-escalation and practical parenting strategies. You are responsible for maintaining accurate records, promoting safeguarding, and collaborating with various stakeholders to ensure improved outcomes for families.
