Who You Are
You are a qualified and experienced Social Worker committed to making a difference in the lives of individuals aged 67 and above with disabilities, mental health issues, and age-related support needs. You must hold a relevant Social Work qualification and be registered with Social Work England. A full driving licence and access to a vehicle are required.
What the job involves
As part of the Later Life Team at St Helens Borough Council, you will work with individuals to undertake Social Care Assessments, identify eligible needs and develop care plans. The role involves identifying carers, completing Joint Carers Assessments, and acting as an Enquiry Practitioner for Safeguarding Adult cases. You will apply the Mental Capacity Act in Best Interest Decisions.
