What the Job Involves
The role involves supporting the delivery of health and social care services in Wirral, working closely with multidisciplinary colleagues and external partners. Your responsibilities include conducting complex assessments, supporting safeguarding processes, developing care plans, and leading multi-disciplinary meetings. You will also help prevent hospital admissions and support adults and families, especially in the areas of mental health, physical health, and aging, while adhering to professional standards and ethical guidelines.
Skills
- Social work qualification and registration with Social Work England
- Experience with vulnerable adults and complex needs
- Knowledge of The Care Act 2014, Mental Health Act, and Mental Capacity Act
- Strong assessment, planning, and safeguarding skills
