Role
Who You Are
A passionate and qualified Senior Social Worker/Social Worker/ASYE with experience in undertaking Social Care Assessments, and working with individuals aged 67+ with physical, sensory disabilities, mental health problems, and dementia.
What the job involves
Working within the Later Life, Disability, Mental Health, DOLS and Review Team to support individuals by identifying eligible needs and outcomes, planning care processes, identifying carers, completing Joint Carers Assessments, and acting as an Enquiry Practitioner in Safeguarding Adult cases.
Skills
- Social Work qualification (CQSW, DIPSW, CSS)
- Registration with Social Work England
- Experience with Mental Capacity Act
- Full driving licence and access to a vehicle
- Experience in safeguarding and care assessments
