What the Job Involves
As an Experienced Social Worker in Adult Social Care, you'll operate within a team to provide guidance, assess needs, and implement social care plans for adults with complex health needs and their carers. You will support the Multi Agency Safeguarding Hub, assess best interests regarding deprivation of liberties, and ensure effective communication among stakeholders. Your responsibilities include managing risk assessments and fostering multidisciplinary collaboration aiming at preventing hospital admissions. You will also undertake professional case conferences and support community initiatives, providing safeguarding responses and leading educational efforts within the service framework.
