Who You Are

We are looking for an experienced social worker to join our Integrated Discharge Team at Whiston Hospital. You should be able to handle a fast-paced, high-pressure environment and have a strong background in facilitating hospital discharges.

What the Job Involves

This full-time position (37 hours per week) involves assessing patients for discharge from the hospital and performing tasks associated with ensuring they receive the support needed to return home or transition to another setting. You will work a mix of office and home-based shifts on a rota basis, and conduct ward visits to assess patients as needed.
