Who You Are
You are an experienced social worker ready to join a busy Adults Community/Locality team. You are flexible with the ability to work hybrid, accommodating mandatory office days on Wednesdays and Fridays, and training/duty days as required.
What the Job Involves
Your main responsibilities include completing social care needs assessments, support planning, and commissioning. You will manage and assess risks, attend MARAMS safeguarding meetings, and be involved in Mental Capacity Act/Best Interests and CHC - DST processes. You are dedicated to providing supportive and comprehensive care to adults in the community.

- Car driver essential
- Experienced social worker
